If finasteride is on your radar for male pattern hair loss, you’re likely facing a deceptively simple question: should you take oral finasteride or use a topical? In practice, that question usually distills down to one concern: which regrows more hair?

In this article, we’ll unpack the differences between topical and oral finasteride, compare the two head-to-head, and examine hair regrowth outcomes, empowering you with the information to make informed decisions when treating your hair loss.

What Is Finasteride?

Finasteride, in its oral form, is an FDA-approved, prescription medication for treating pattern hair loss. Its primary function is lowering a male hormone called dihydrotestosterone (DHT) and is typically prescribed at a daily dose of 1 mg to treat androgenic alopecia (AGA).

DHT is an androgen (or male hormone), produced from testosterone by the 5a-reductase enzyme. It plays a crucial role in male pattern hair loss (also known as androgenic alopecia) – causing hair loss when it increases in scalp tissues. It does so by interacting with genetically sensitive hair follicles in the temples and crown, setting the development of pattern hair loss in motion.

Finasteride lowers DHT levels by inhibiting the 5a-reductase enzyme, lowering the conversion of testosterone into DHT.

When taken orally, finasteride travels through the blood stream to the scalp, effectively reducing scalp levels of DHT. As a result, finasteride protects vulnerable hair follicles from DHT’s negative effects and slows, stops, or even reverses the development of patterned hair loss.

What Is Topical Finasteride?

When finasteride is taken orally, its DHT-lowering effects aren’t just localized to the scalp – they’re systemic. That means oral finasteride lowers DHT in the blood, brain, and testes.

nd because DHT plays a significant role in male physiology, DHT reduction in these tissues means that it has the potential to interfere with various DHT-related aspects of health – most notably in the areas of sexual health and cognition.

Not surprisingly, sexual and cognitive side effects are a risk when taking finasteride. Even though the overall risk remains low – less than 2% – sexual health and cognition are an area of high priority. As such, even a low risk of related side effects can be a significant concern for men interested in using finasteride.

As a result, the desire to preserve finasteride’s effective DHT-reducing capabilities while limiting systemic exposure led to a natural evolution in treatment design: the development of topical finasteride. By delivering the DHT-lowering effects of finasteride directly to the scalp, researchers hoped to minimize the impact to systemic DHT levels and, thereby, the risk of side effects.

Is Topical Finasteride Effective?

Over the past two decades, topical finasteride formulations have emerged and research has followed with a clear observation: topical finasteride does regrow hair and does so at concentrations between 0.005% and 1%.

More importantly, topical formulations improve hair outcomes while minimizing impact to systemic DHT. However, this is dose dependent:

  • Low dose topical finasteride (0.005%, 2 mL daily) regrows hair without any appreciable impact to serum DHT levels.
  • Standard dose topical finasteride (0.25%, 1 mL daily) lowers scalp DHT by 70%, but also lowers serum DHT by as much as standard oral finasteride.
  • 0.25% finasteride, dosed at 0.05 - 0.2 mL daily, reduced serum DHT by 34.5% compared to 55.6% with 1 mg oral finasteride, representing a middle ground between the two doses.

From the results of this research, we can conclude two important things:

  1. Systemic DHT reduction isn’t a prerequisite for hair regrowth. That is, it’s possible regrow hair just by lowering scalp DHT.
  2. Topical finasteride accomplishes its goal: hair regrowth while minimizing systemic impact to DHT.

With that background established, we can move to a more pointed question: how does it compare to oral finasteride? More importantly, which regrows more hair – oral or topical finasteride?

Topical Finasteride vs. Oral Finasteride: Which Regrows More Hair?

Several studies have sought to answer the question: which regrows more hair – topical or oral finasteride? In the head-to-head comparisons, results are mixed.

Study #1: 0.25% topical finasteride, 0.05-0.2 mL daily vs. 1 mg oral finasteride

In a 2021 study, researchers found 0.05 to 0.2 mL of 0.25% topical finasteride, applied once daily, was effective for increasing hair counts. This equates to a daily exposure of approximately 0.1 to 0.6 mg of finasteride.

When compared to 1 mg of oral finasteride daily, the topical finasteride dosing regimen produced similar hair regrowth.

More importantly, it did so with markedly lower systemic exposure: blood levels of finasteride were over 100x lower in the topical group vs. the oral group. Serum DHT reduction was also minimized, with topical finasteride lowering serum DHT by 34.5% versus 55.6% in the oral group.

Taken together, these findings suggest that relatively low daily doses of topical finasteride can deliver hair growth outcomes comparable to oral finasteride, while appreciably limiting systemic exposure.

Despite achieving similar improvements in hair counts, topical finasteride resulted in dramatically lower circulating finasteride levels and a more modest reduction in serum DHT.

Study #2: 1% topical finasteride vs. 1 mg oral finasteride

In a 2009 study, researchers compared two formulations of finasteride: a 1% topical gel and 1 mg of oral finasteride daily. At the end of the study, both groups achieved similar levels of regrowth. However, the oral finasteride group achieved faster regrowth than the topical group.

Study #3: 0.25% topical finasteride, 1 mL daily vs. 1 mg oral finasteride

In one study, participants applied 1 mL of a 0.25% topical finasteride solution once daily and were compared to those taking 1 mg of oral finasteride daily.

1 mL of 0.25% topical finasteride equates to 2.5 mg of finasteride applied daily. So, compared to the oral finasteride group, the topical finasteride group used a 2.5x higher daily dose of finasteride.

This study differed from the others in one important way: it did not measure hair regrowth directly. Instead of tracking changes in hair counts, researchers focused on scalp DHT a useful proxy for finasteride’s local activity (and possibly efficacy).

And by that measure, topical finasteride performed strongly. Scalp DHT was reduced by roughly 70% with topical application, compared to about 50% with oral finasteride. That difference may be explained by more concentrated delivery at the scalp, the higher applied dose, or both.

However, at this dose, topical finasteride reduced serum DHT levels to a similar degree as oral finasteride. This suggests that while a 2.5 mg daily dose of topical finasteride might be better at decreasing scalp levels of DHT, it still reduces blood DHT levels as significantly as 1 mg oral finasteride.

So, while it may be better at reducing scalp DHT – potentially setting the stage for greater hair gains – it may not have a better risk profile than oral finasteride.

The Verdict: Does Topical or Oral Finasteride Regrow More Hair?

Collectively, research is still limited. However, we can observe a few patterns in the small body of research:

  • Topical finasteride seems to regrow hair at a similar rate to oral finasteride. This seems to hold true whether the topical dose is more or less than 1 mg – the dose typically used orally.
  • This is likely a result of greater bioavailability at the scalp level. In other words, topical finasteride is better at delivering finasteride where it works to regrow hair: to the scalp.
  • Oral finasteride only outperformed topical finasteride in one study – not for the amount of regrowth achieved, but for the speed at which hair returned.
  • At a certain dosage, topical finasteride may outperform oral finasteride in its ability to reduce scalp levels of DHT. However, these dosages likely lead to more significant systemic exposure and comparable reductions in serum DHT (and, as a result, a similar risk profile to oral finasteride).

By and large, no formulation regularly outperforms the other. Oral and topical finasteride seem to be similarly effective for regrowing hair, but more research is needed to be definitive.

The Deciding Factor: Topical or Oral Finasteride for Hair Regrowth

Because oral and topical finasteride seem to be on par in terms of efficacy, the deciding factor in choosing one over the other is not effectiveness. Instead, it’s likely your treatment preferences:

  • Your preference for evidence. If you’re looking for robust clinical data to support your treatment choices, oral finasteride clearly outperforms topical finasteride. There is decades of data to support oral finasteride’s use and comparatively less for topical finasteride.
  • Your comfortability with the risk of side effects. If you’re looking to minimize the risk of finasteride’s potential side effects while still addressing DHT, topical finasteride (especially low dose finasteride) offers similar efficacy while minimizing the systemic impact of finasteride.
  • Your lifestyle. Do you prefer the convenience of a once-daily pill? If so, oral finasteride might be a better fit for your lifestyle. 

Finasteride for Hair Regrowth: The Bottom Line

Across multiple studies, topical finasteride has reliably shown the ability to regrow hair, often to a degree comparable to oral finasteride. In some cases, it achieves these results with dramatically lower systemic exposure, offering a viable alternative to oral finasteride that minimizes the risk of side effects.

So, which regrows more hair? Based on the evidence available today, neither form clearly outperforms the other in terms of overall regrowth.

Instead, topical and oral finasteride appear to be more alike than different: two effective ways of targeting the same biological process, with meaningful trade-offs in evidence, systemic exposure, and lifestyle fit.
